Our Life Care Plans typically include:

A life care plan is a comprehensive document that identifies the continuing medical conditions and ongoing care requirements of individuals who have experienced a catastrophic injury or have chronic health care needs. This dynamic document is built on established rehabilitation principles, a thorough methodological analysis of extensive patient data, and an in-depth interview with the client or caregivers to understand how this injury or illness is impacting the client’s life on a daily basis . It quantifies the costs of supplying these individuals with medically-related goods and services throughout an estimated life span. The goal of this powerful tool is to minimize possible complications as well as maximize functional potential for the individual.

Using the five steps of the nursing process, life care planners at Mitstifer & Co create these specialized documents.

  1. Perform comprehensive assessment of the individual’s residual medical conditions and identify areas of concern.
  2. Conduct interviews in person or via remote meetings and collaborate with other members of the medical team to better understand how the client’s life has been impacted.
  3. Extensive cost research into local resources of the individual obtained utilizing usual, customary, and reasonable (UCR) charges.
  4. Provide supporting evidence and validity of the individual’s medical conditions.
  5. Produce a narrative with careful consideration of the goals, needs, and interest of the client, and the needs of the family for future care related to the injury or illness. This process is based upon the current pattern of care, collaboration with healthcare professionals, evidence-based clinical standards, and the knowledge and expertise of our experienced nurses.

When an extensive life care plan is unnecessary, a medical cost projection report continues to provide estimated future medical costs for use in mediation, setting accurate reserves, and in settlement negotiations. Typically, recommendations for medical surveillance, interventional pain management, and/or surgical interventions are provided. Our life care planners prepare this invaluable tool by reviewing medical records and recommendations from treating and/or consulting physicians. This concise and expedited form includes estimated costs for relevant medical and pharmacological needs for an annual basis as well as an estimated lifetime.
